Advancing Practitioners, Inc. (d/b/a “AP Health” and referred to as the same, “we”, “us”, and “our” in this Privacy Policy), accessible from aphealth.com, is a surgeon-centric outsource provider of surgical care staffing for hospitals.­

This Privacy Policy document contains the types of information that is collected and recorded by us and how we use it.  This Privacy Policy applies only to our website.  This Privacy Policy does not apply to the practices of companies or affiliates that we do not own, or to individuals whom we do not employ, manage, or work with, including operators of websites that you may visit before, during, or after visiting our website.  We recommend that you view privacy policy statements of any other companies, individuals, or organizations that may interface with AP Health in order to understand how they may collect, use, and share your information.

This Privacy Policy is not a contract or a legally binding agreement between you and AP Health.  However, by using our website, you hereby consent to our Privacy Policy.

What Information Does AP Health Collect Through Its Website and How Does AP Health Use That Information?

We may collect certain non-personally identifiable information automatically through our website using cookies, web beacons, and tracking codes to monitor website traffic and user activity, as well as to help us to understand usage and marketing effectiveness.  We also use log files, which log visitors when they visit websites.  Many hosting companies use log files and are a part of the hosting services’ analytics.  The additional information collected about you through these technologies includes internet protocol (“IP”) addresses, browser type, Internet Service Provider (“ISP”), date and time stamp, referring/exit pages, your computer operating system, your general location (not exact address), and possibly the number of clicks.  This information is not personally identifiable and generally cannot become identifiable even when aggregated.  We use this information to analyze trends, administer our website, track users’ movement on the website, and gather demographic information.

We also collect information that you provide directly to us, usually through a “Contact Us” form, which includes but is not limited to:

We do not directly accept credit card payments through our website.  Where you are invited to pay bills or invoices or purchase merchandise through our website, those transactions are carried out by third parties.  Therefore, we advise you to review those third parties’ security and privacy policies prior to completing any transaction using their payment services.  You are welcome to pay our invoices without using a credit card through EFT (electronic funds transfer) or by sending a check to our address of principal place of business.

Sharing of Information.

AP Health does not intentionally share or communicate personally identifiable information to any unrelated or unaffiliated third party.  AP Health does not sell any of your information.  However, we may communicate the information collected through our website or the information you provide to us to authorized employees and contractors within our business, and contractors, agents, and entities with whom we work to help provide marketing or advertising material to you or to deliver our services to you.  Examples of authorized agents or entities to which we may need to give access to your information include companies we have hired to provide our Internet services and to protect and secure our servers, website, or computer systems.  We may also need to disclose your personal information as part of a corporate transaction such as a merger or sale of assets.

Information We Receive from Third Parties.

We may receive information about you from third parties in the industries in which we operate, such as industry tradeshows or conferences, through social media, or through medical or healthcare providers.  If you attend industry tradeshows or disclose your information on another website, for example, and opt-in to receive information from us, then these third parties will submit to us your e-mail address and other contact information about you so that we may contact you as requested.  You may also choose to participate in a third-party application or feature (such as one of our Facebook or LinkedIn pages) through which you allow us to collect information about you, including your name, location, usage information, and personal information, such as “likes” or comments you share.  You agree and understand that we have limited control on information you provide through such third-party applications and features.

Your Privacy Rights, Choice, and Access.

You may not be able to control the non-personally identifiable information disclosed to us and other websites through your browser and ISP.  We cannot control the means through which this information is delivered or how it is communicated as such information is necessary to provide basic networking functionality.  For example, third-party servers use technologies like cookies, JavaScript, and web beacons; and the files associated with these technologies are sent directly to users’ browsers.  The third-party servers automatically receive your IP address when this occurs.  AP Health has no access to or control over these cookies that are used by third-party servers or advertisers.  Additionally, we would have trouble identifying which information belongs to you even if we were to have the ability to remove such information.  Therefore, we will decline or not respond to any request to remove non-personally identifiable information associated with such technologies from our website or computer systems.

 

You generally are able to control on the other hand the personally identifiable information that you provide directly to us.  If you wish to modify, verify, correct, or delete any of your personal information collected through our website or services, then you may contact us with your instructions.  Due to the nature of technology and websites, we cannot promise we can remove, delete, or return every trace of information or data you provide to us, if you make such a request.  Furthermore, we will retain and use any and all information necessary to the extent to comply with our legal obligations, resolve disputes, enforce our agreements; and protect our rights, property, and safety.  However, we will attempt to work with you and attempt to comply with every reasonable request you may make regarding the control of your personal information.

 

In accordance with our routine record keeping and compliance with certain federal privacy laws, we may also manually delete certain records that contain personally identifiable information that you have submitted to us and certain records containing your personally identifiable information may be deleted automatically.  We are under no obligation to store such personally identifiable information and disclaim any liability arising out of or related to the destruction of such personally identifiable information.  We are under no obligation to notify you if and when your personally identifiable information is deleted or removed from our records.

 

We do not control certain privacy settings and preferences maintained by social media, like Facebook, LinkedIn, and Twitter.  If you wish to make changes to those settings and preferences, you may do so by visiting the settings page of the appropriate social media website.

We use standard industry digital security mechanisms to help protect your information, but please note that no electronic data transmission or storage of information is infallible and 100% secure.  Therefore, we cannot ensure or warrant the absolute security of any information you transmit to us, and you consent to our services and provide us with your information with that risk in mind.

Changes to This Privacy Policy.

We reevaluate our current privacy and information security processes every so often and may update or change the scope of this Privacy Policy to reflect changes in our information management practices and evolving technology, laws, and business practices.  If we make any material changes, we will notify you by means of a notice on our website or possibly by an e-mail address, if you disclosed it to us.  We reserve the right to amend this Privacy Policy with nonmaterial changes without giving any notice to you, so we encourage you encourage you to periodically review this page for the latest information on our privacy practices.

Children’s Online Privacy Protection Act (“COPPA”) Disclaimer.

We do not advertise to children under the age of 13 nor do we knowingly collect any personally identifiable information from children under the age of 13.  If you think that your child provided this kind of information on our website, we strongly encourage you to contact us immediately and we will do our best efforts to promptly remove such information from our records.
